Dr. Kimberly Monday Leaving HNI to Start New Role at UT Neurology Clinic

Dear Valued HNI Patient,
After 20 wonderful years leading HNI, I have made a difficult decision to accept a new position within the
University of Texas McGovern Medical School. This is a full-time position in the Department of Neurology as Vice
Chair of Clinical Operations. As part of this position, beginning in November, I will dedicate 2 days a week to
seeing outpatients in the UT neurology clinic, which is located at 6410 Fannin Street in the Texas Medical Center.
I understand that many patients will prefer to remain with HNI. We recently added Myrtle Jeroudi, M.D. to the
HNI Group. My wonderful PA, Jessica Hanson, will continue to see patients in both Pearland and Pasadena. In
addition, Drs. Eberle, Fayle, Palvadi, and Fankhauser will maintain their current schedules. I would entrust my
family's care to anyone in our group. With the addition of Dr. Jeroudi, HNI will have the capacity to take care of
any of my patients who prefer this option.
Liz Degollado at the HNI front desk (713-947-3100) has been tasked to help you reschedule with another HNI
physician or Jessica Hanson, PA (with another HNI doctor as back-up). Liz will also be facilitating the coordination
of medical records to transfer to UT Neurology for those who want to move their care to the Medical Center.
To make a new patient appointment with me at the University of Texas at Houston Department of Neurology
call 832-325-7090 and speak with Emily, Sylvia, or Tamika. They will help you enroll in the UT system and
schedule an appointment with me for a Monday or Wednesday at 6410 Fannin Street beginning in November.
There may be some insurance plans that differ with respect to referral needs. Both offices will help you navigate
those requirements.
